This notice describes how medical information about you may be used and disclosed and how you can get access to this information.
Referral Form
Provider referral for Psychological and/or Neuropsychological testing.
This form can be sent to palmetto psychological via email (referral@palmettopsych.com) or via fax (864-529-9833).
Referrals are not required but helpful if the client is planning to file an Out-Of-Network (OON) claim to their insurance plan for reimbursement of fees paid.
